Step-1 Please take printout of the ST1 form as shown on the bottom of the page.
Step-2 Mention your PAN,name,address, and other miscellanous particulars,etc and enclose the copy of the PAN card,copy of the Partnership deed (in case of a firm) and the copy of Memorandum/Articles of Association in case of a Company and the copy of address proof.
Step-3 You are required to make the payment of Rs 3500/- online via credit card/debit card/online banking/visa/master card on the link shown as down below and then to send us the aforesaid documents via fax or email.
